
It has been over a week since I last posted to this blog, an unusually long time for me, frustrating but unavoidable.
My time has been spent compiling the fourth and final part my submission of evidence to support the full scope of my ‘fitness to practice’ complaint against Health and Care Professions Council registrant, Sally Mungall.
‘PART FOUR Summary of FULL SCOPE of complaints against HCPC registrant, Sally Mungall’ concludes a comprehensive and detailed submission of new evidence alongside evidence previously submitted to the HCPC. I have correlated all evidence against thirteen categories and subsets of the HCPC ‘STANDARDS OF CONDUCT, PERFORMANCE AND ETHICS’ that I believe have been infringed by the registrant. I have also submitted a 19-page summary of my ‘art therapy’ with Sally Mungall couched in an account of what happened before and after.
My ‘fitness to practice’ complaint is scheduled for assessment with the HCPC Investigatory Committee Panel on the 11th February, 2026. I will wait until after the assessment before making a decision to publish everything here at Four and Twenty Dead Crows, though it may be wiser to allow the ‘fitness to practice’ process to run its complete course before publishing anything else.
THE PREVENT REFERRAL DEBACLE
The next thing I need to do is compile all evidence relating to the preposterous referral of me to the government’s anti-terrorism program, PREVENT. My efforts to access records held by those involved in that referral, ie the Safeguarding Board and Information Governance at Hampshire and IOW Healthcare Trust and Public Access, JIMU, Hampshire & Isle of Wight Constabulary & Thames Valley Police, has been cynically resisted. Sadie Bell at the Trust and Jason Russell at JIMU have been playing ping pong with the evidence that I need, leaving me at the net waiting for the Information Commissioner’s Office to intervene. I had previously been advised by the ICO that there was a waiting time of 29 weeks for new complaints.
Fortunately, the ICO have intervened much sooner, acknowledging the deadlines involved bringing this issue to the attention of lawyers. If I am to pursue legal action against the Trust then I need to present evidence to lawyers affiliated with PreventWatch, ideally before the end of this month.
I received an email from Paula Flanagan, Lead Case Officer at the ICO on the 15th January with reference to IC-*********** Hampshire Constabulary and IC-*********** Hampshire & IOW NHS Foundation Trust assuring me that ‘I am giving your concerns my urgent attention.’
Then, on 16 January 2026 I received the following email

Good morning, Mark
I am writing to confirm that I have written to Hampshire and IOW NHS Foundation Trust this morning about the way they are handling your personal information and your ongoing data protection concerns.
We would normally allow a data controller one month to respond to our enquiries, but in your case I have asked the Trust if they could respond to us by 30 January. Please note however that this may not be possible as we are conscious that their Information Governance team is currently handling a large volume of data protection related matters. We will wait and see if they can agree to this short deadline.
If I am able to provide you with an interim update in the meantime, I will of course do so.

While I wait on the outcome of Paula’s intervention I will go ahead and compile the evidence already disclosed to me and make a robust argument in writing for legal proceedings against the Trust.
The Prevent duty guidance: England and Wales (2023) https://assets.publishing.service.gov.uk/media/65e5a5bd3f69457ff1035fe2/14.258_HO_Prevent+Duty+Guidance_v5d_Final_Web_1_.pdf sets clear expectations for each of the main statutory sectors and describe the ways in which they should comply with the Prevent duty. The argument will rest on proving MISCONDUCT by the individual referrer as well as ignorance and incompetence and misuse of the Prevent duty guidance by the Safeguarding Board of the Trust. Complaints about the whole referral to PREVENT, including the implied involvement of the police, will be formally presented through the government complaint process at https://www.gov.uk/guidance/make-a-complaint-about-prevent .
When this work is complete it will be considered by lawyers affiliated with PreventWatch.
