The event, both cynical and vexatious, that played out on the 25th of September, 2022 was an inciting incident so compelling that it changed my attitude towards CAMHS art therapist, Sally Mungall, demolishing an ideal perfectly graven in my heart and replacing it with disillusion and disappointment. The countdown to demolition ticked silently as August gave up its last breath.


Andy Ashby’s response to my complaint had rendered me frustrated and angry. I took my complaint to the Parliamentary Health Service Ombudsman, completing their online forms on the 31st August, 2022.


2nd September 2022. T-Minus twenty-three days and counting.  

I forwarded an access to records request to the Information Management Team at Hampshire Constabulary. My medical records had confirmed that Sally Mungall had been advised to report me to the police and I desperately wanted to know what mischief had been caused. S Adcock, a Public Access Assistant,  informed me that two forms of I.D. were required alongside written consent from the third party involved. I immediately returned to Andy Ashby and requested third party consent.


3rd September 2022. T-Minus twenty-two days and counting. I made a request of Katie Rees, Head of Information Governance at the Sussex Partnership  for access to ALL records relating to my ‘parent work’ sessions with Sally Mungall.

It was Andy Ashby, the Heading of Nursing working at PALS SP that I asked assistance from. It was Andy Ashby who compiled the response to my complaint against Camhs, Bramblys Drive. The response included the reference to ‘actions taken to contact the police’ under item ‘10’ of the response ( full wording in my email to you in thread below ). Very serious unfounded allegations were made against me at the time of the Safeguarding meeting and possibly two more meetings that took place at around the 1st February 2022. Those unfounded allegations, along with further incidences of negligence and clinical malpractice have caused me significant psychological damage that was foundational to a suicide attempt made by me in April this year. I am still suicidal. I made my original access to records via post sent to your office on the 23rd April 2022. I understand and accept that IG SP experienced significant IT issues and am grateful for the work that you have put into delivering my records. The truth is, however, that records have been drip-fed to me by Wanda in what I suspect to be a deliberate tactic to slow down and obstruct my investigation. For example, I only received confirmation of details of a Safeguarding Internal Case Discussion because you kindly spent time looking for additional documentation for me even though I hadn’t asked ( sent to me on 9th August 2022 ). If Wanda was truly cooperating with me then she would have made this document available to me along with the rest of SPFT MAY22-021-REP on 21st June 2022. And now there appears to be evidence that the police were contacted and unfounded allegations made against me following the Safeguarding Meeting. I have been in contact with Hampshire Police yesterday to make an application of my own personal data to determine details of Camhs allegations. I have been advised that I need written consent from those involved at Camhs before they can disclose full, unredacted data. I have asked Andy Ashby to ask for that written consent. I do not expect Camhs to cooperate. While they have admitted to malpractice and apologised for the distress that they have caused they are still withholding much information. I absolutely need to know as much as possible in relation to Camhs contacting the police after the Safeguarding meeting. I also need to supply PHSO with all the pertinent documentation, including digital copies of Sally Mungall’s session notes that she kept during the so called ‘parent work’ that was delivered to me between 2nd Sept 2021 and 16th December 2021. These are the same ‘session notes’ that Wanda printed off for the meeting with me on 20th May 2022. Those ‘session notes’ prove that I was given inappropriate and psychologically damaging psychotherapy. Wanda and PALS SP have already admitted that this was a mistake caused by Camhs but I need PHSO to consider the same ‘session notes’. I would be very grateful to you if you could process these and send them to me by this Wed morning if possible. That leaves me with ‘I don’t know what I don’t know’, meaning that there could still be documents in the system relating to me that I don’t know about and so cant make specific requests for. For example I managed to surmise that there have been a possible two additional meetings held at Camhs, Bramblys Drive at around the same time as the Safeguarding meeting. I only found out about those additional meetings following your disclosing the Safeguarding Internal Case Discussion on 9th August 2022. It appears that they didn’t record minutes for either of those meetings, deeming them ‘operational’ and not requiring record-keeping ( which I consider a serious oversight ). What other records were either not made or not entered into the ‘system’ ? I would never had been aware of any of the malpractice or negligence had I not decided to make Access to Records at both Camhs ( Sussex Partnership ) and the Bridge Centre ( Southern Health ). I was being lied to, deceived, emotionally manipulated, pathologised and potentially criminalised and All of this was happening behind my back. I count myself fortunate to have had you on my side.

5th September 2022. T-Minus 20 days and counting. I received an email from Andy Ashby informing me ‘I have forwarded your request onto the appropriate Head of Services last week but have not heard back as yet…’


7th September 2022. T-Minus 18 days and counting. I emailed Julie Yalden, the named nurse for Safeguarding Hampshire CAMHS, asking for an explanation for decisions made during the SAFEGUARDING-INTERNAL CASE DISCUSSION held on the 1st February, 2022. 9th September 2022. T-Minus 16 days and counting. I received an email from Andy Ashby informing me ‘I have spoken to Katie and she has kindly agreed to provide you a response to your request below’. This was in response to my request for third party consent to access records held by Hampshire Police. This was roundly ignored.


15th September 2022. T-Minus 10 days and counting. I was informed by Farayi Nyakubaya, Head of Nursing at CMHT the Bridge Centre that Meg had been allocated a new care coordinator, Clem Feeney, and that psychologist, Dr Vizzini was scheduled to assess Meg on the 27th September.


16th September 2022. T-Minus 9 days and counting. Following a further request from my GP, Dr Dougan, I attended the first of two mental health assessment appointments with consultant psychotherapist, Dr Isa Shafii.

Also on this same day I received an update from the PHSO informing me that ‘…you are likely to wait up to between 9 to 11 months before we can allocate your case to a caseworker.’

 

The update from the PHSO left me feeling desperate.
