The following is a document intended for widespread distribution on the 30th September, 2025.
MISREPRESENTATIONS, FALSE ALLEGATIONS AND OMISSIONS IN MY MEDICAL RECORDS
On the 9th September, 2025, I received documents following my latest access to medical records request from Larissa Cleverley at Southern Health NHS Foundation Trust ( now Hampshire and IOW Healthcare ). My access to records request was made to support my investigation into the recent inappropriate referral of me to Prevent, the government’s counter terrorism initiative, by former Head Of Nursing, Gemma Stubbington, endorsed by the Safeguarding Board of the Trust. The documents were separated into four folders and I initially focused on the third and fourth which contained the information relevant to Prevent.
Over the last few days I have taken the opportunity to check the first and second folder and am horrified to discover numerous misrepresentations of me, along with false allegations and an unexplained omission of records. I had previously identified numerous misrepresentations, false allegations and actual lies made about me by clinicians and leadership at CAMHS, Bramblys Drive, Basingstoke and the wider Sussex Partnership NHS Foundation Trust and disputed other contentious entries made by clinicians and leadership at CMHT, The Bridge Centre throughout 2022. I had addressed these misrepresentations and false allegations in a series of formal complaints and had expected healthcare professionals to exercise caution with their written statements about me in future.
Lessons have evidently NOT been learned.
I accept that many of the inaccuracies in my medical records are attributable to carelessness in recording and are considered by me to be relatively insignificant. There are, however, three separate issues that I want investigated and for action to be taken.
The three separate issues are as follows
1) The omission from the records of details of the CPA meeting held at CMHT, The Bridge Centre at 1pm on the 6th June, 2024.
2) The statements made by Nicola Hoyle during a 'Professional Meeting' at CMHT on the 30th April, 2024.
3) The prevalence of an unqualified statement about me made during that same ‘Professional Meeting’ at CMHT which has been used to describe me in every subsequent MDT meeting where I have been the subject of discussion.
In detail
1) I was invited to attend a CPA meeting at 1pm on the 6th June, 2024 at CMHT, The Bridge Centre. I was informed that this meeting would be attended core care staff alongside Dr Joshua Olofinlade and my care coordinator at the time, Fred Amanfo.
When I entered rm 13 with Fred I was greeted by Dr Joshua Olofinlade. There wasn’t anyone else in the room. There were no ‘core care staff’ in attendance. I was introduced to two ladies in attendance via Zoom who were there to make an administrative record of the meeting. I was then introduced to PC James Charlton of Hampshire Police, also in attendance via Zoom.
At that point it was evident that I had been invited under false pretence. I was AMBUSHED by PC Charlton. I had no warning of his attendance and was completely taken by surprise, having been afforded no opportunity to compose myself or prepare. I reacted immediately with extreme anger.
I took the opportunity to berate PC Charlton and the monstrous and dishonest way that Hampshire Police had treated me before giving him permission to remove himself from the meeting.
I then turned to Dr Olofinlade asking him to explain the deception, the absence of core care staff and to explain the ambush by PC Charlton. He couldn’t offer an explanation for either. I then made apologies to the two ladies and left the meeting. Fred chased me down the corridor and I spoke with him for a few moments. Fred assured me that he had not been responsible for the deception.
Nobody has ever taken responsibility for the deception although the incident has reinforced my opinion that Dr Olofinlade is incompetent and profoundly dishonest.
While there IS record of my stated dissatisfaction over the CPA incident in telephone conversations with shared care staff in the days following, there is absolutely NO record of the meeting itself. I am assuming that details of the meeting were NOT entered into my medical records because doing so would record the grievous mistakes made those responsible.
WHY WERE THE DETAILS OF THIS MEETING NOT ENTERED INTO THE RECORD?
AND, WAS DR OLOFINLADE RESPONSIBLE FOR THE DECEPTION?
2) NICOLA HOYLE, in a ‘Professional Meeting’ held at CMHT, The Bridge Centre on the 30th April, 2024, made the following statements
‘Mark’s involvement with his therapist (Sally Mungall) , ‘was also discussed in this meeting.’
The record is conspicuously absent of any details of what exactly was ‘discussed’ though I am certain beyond reasonable doubt that I was misrepresented by Nicola Hoyle. I am certain of this because of Nicola Hoyle’s previous involvement in my case, especially during a previous ‘professionals meeting’ held on the 8th February, 2022.
This meeting, along with all meetings from this point on start now start with a description of me, the second line always being,
‘… Mark was reported to the police for stalking a member stalking a member of staff who was involved in his care.’
Who initiated that descriptor? It was never used before this meeting. It has now been used in every MDT meeting held at CMHT when referring to me.
WHO WAS IT THAT FIRST INTRODUCED THIS DESCRIPTOR, ‘WAS REPORTED TO THE POLICE FOR STALKING A MEMBER OF STAFF WHO WAS INVOLVED IN HIS CARE’?
NICOLA HOYLE?
Did Nicola Hoyle make this claim against me while embellishing my ‘involvement with his therapist?’
NICOLA HOYLE MAKES A FURTHER STATEMENT RECORDED AS
‘Nicola Hoyles (sic ) shared in the meeting that in the past professionals meeting she attended regarding Mark Stoke ( sic ) it was reported that Mark had made a pact with her (?) daughter to end their life by suicide if their demand from the services were not met.’
‘WHO ORIGINALLY FABRICATED THE STORY OF A SUICIDE PACT BETWEEN MY DAUGHTER MEG AND ME?!!’
Did this fabrication originate with Sally Mungall?
Precedence had already been set as records already confirm that Sally Mungall had been consistently dishonest while reporting on me.
Did Sally Mungall mislead Nicola Hoyle on this particular matter?
Or was this story a farcical embellishment concocted entirely by Nicola Hoyle?
Or a particularly appalling and highly inflammatory case of Chinese Whispers?
Nicola Hoyle knows the truth but I wouldn’t trust Nicola Hoyle’s word on anything.
My medical records confirm the following
Sally Mungall stated in telephone call to Ian Whaites on 31st January 2022 that
‘ Mark is desperately get support for his daughter, in case anything happens to him, so that she’s looked after’
Sally Mungall wrote the following on the 1st February, 2022 as part of an invitation to ‘professionals meeting’ on the 8th February, 2022
‘Consider the potential risks for Meg – Megs life is taken.’
A statement made by Nicola Hoyle during that same ‘professionals meeting’ held on the 8th February, 2022 in answer to Julie Yalden’s (CAMHS ) question ‘…assuming we are not looking at the risk of dad harming Meg in the process of ending his own life…’ replied
‘…no, reads that he is looking for further provision of care for meg.’
Evidence would suggest that everyone was fully aware that I considered my daughter’s safety to be of paramount importance. Where is the original statement made by anyone that I have made a suicide pact with my daughter?
Nicola Hoyle went further during that ‘professionals meeting’ on the 8th February, 2022 and suggested, incorrectly, that I had ‘crossed boundaries’ where none had been drawn or identified. She then made the unsubstantiated claim that I was a risk to Sally Mungall and recommended that the police were called.
FOR THE RECORD, THERE WAS NEVER ANY SUICIDE PACT MADE BETWEEN MY DAUGHTER, MEG AND ME. IT IS A RIDICULOUS AND COMPLETELY UNFOUNDED ALLEGATION. I AM OUTRAGED THAT THIS ACCUSATION WAS MADE AND WAS ALLOWED TO REMAIN IN MY MEDICAL RECORDS UNCHECKED.
I WANT THIS UNFOUNDED ALLEGATION INVESTIGATED.
I WANT NICOLA HOYLE TO ACCOUNT FOR HER ALLEGATION. I RECOMMEND THAT THE TRUST EITHER APPROACH HER DIRECTLY OR ADVISE HER TO MAKE CONTACT WITH ME DIRECTLY OR TO AUTHORISE CONTACT DETAILS SO THAT I CAN CONTACT HER DIRECTLY.
3) ‘… Mark was reported to the police for stalking a member stalking a member of staff who was involved in his care.’
This statement was entered into my medical records following the ‘professional meeting’ held at CMHT, The Bridge Centre on the 30th April, 2024. It had never been used in any meeting before but has subsequently been used to describe me at the beginning of every MDT meeting since. I believe that this description has been adopted following Nicola Hoyle’s account of me recorded as
‘Mark’s involvement with his therapist (Sally Mungall) , ‘was also discussed in this meeting.’
I INSIST THAT THIS DESCRIPTION IS QUALIFIED BY MY ROBUST DENIAL.
I ABSOLUTELY AND ROBUSTLY DENY ANY CHARGES THAT ACCUSE ME OF STALKING SALLY MUNGALL.
I CATAGORICALLY DID NOT STALK SALLY MUNGALL.
THERE IS NO EVIDENCE OF ME EVER STALKING SALLY MUNGALL.
I HAVE REPEATEDLY ASKED HAMPSHIRE POLICE TO PRESENT ANY EVIDENCE THAT THEY HAVE TO SUPPORT ANY REPORT THAT I STALKED SALLY MUNGALL.
NO EVIDENCE HAS EVER BEEN OFFERED TO SUPPORT THIS ALLEGATION BECAUSE THERE IS ABSOLUTELY NO EVIDENCE.
I even challenged the insinuation of stalking made in the recorded conversation between Ian Whaites at CMHT, The Bridge Centre and Weston Henry in February 2022 and secured a written apology.
Gemma Stubbington, former Head of Nursing at CMHT, The Bridge Centre made the following statement in her inappropriate referral of me to Prevent.
‘Mark has complained about police due to an incident named by police as
stalking a health worker'.
I have since written to Gemma asking her to explain her remarks and choices in the
Prevent referral. The following is an extract
‘THIS MAKES ME SO FUCKING ANGRY.
Your words here are a perfect example for the whole reason that I am protesting against Hampshire Constabulary. For as long as Hampshire Constabulary refuse to remove false, discriminatory and psychologically damaging information under my name on their database, that information can be used against me.
I ABSOLUTELY AND CATAGORICALLY NEVER STALKED ANYONE. I HAVE REPEATEDLY CHALLANGED HAMPSHIRE CONSTABULARY TO OFFER EVIDENCE OF ME STALKING. THEY HAVE NEVER OFFERED EVIDENCE BECAUSE THERE IS NO EVIDENCE.
THE FACTS ARE THAT I WAS VISITED BY POLICE AT MY HOME ON THE 25TH SEPTEMBER, 2022 AND OFFERED ‘WORDS OF ADVICE’ ABOUT CONTACTING CAMHS ART THERAPIST, SALLY MUNGALL. APPARENTLY, THE LETTER AND COMPLAINT DOCUMENTS THAT I SENT TO SALLY MUNGALL WERE ‘UNWANTED’. HOW WAS I TO KNOW THAT MY CONTACT WAS ‘UNWANTED’? SALLY MUNGALL HAD NEVER ADVISED ME THAT SHE DID NOT WANT CONTACT WITH ME.
I WAS LATER INFORMED BY PC A LOWE OF PROFESSIONAL STANDARDS HAMPSHIRE CONSTABULARY THAT THE VISIT BY POLICE AT MY HOME WAS NOT A HARASSMENT WARNING. NEVERTHELESS, THE ‘OCCURRENCE’ WAS RECORDED UNDER A HEADING OF HARASSMENT AND STALKING. THIS IN ITSELF IS AN OUTRAGE AS I NEVER HARASSED SALLY MUNGALL AND THERE IS ABSOLUTELY NO EVIDENCE OF ME STALKING SALLY MUNGALL BECAUSE I ACTUALLY NEVER STALKED SALLY MUNGALL.
AFTER EXHAUSTIVE EFFORTS ON MY PART AND AFTER BEING GIVEN THE RUN AROUND BY AT LEAST THREE POLICE DEPARTMENTS FOR OVER A YEAR, A SPOKESPERSON FOR HAMPSHIRE CONSTABULARY CONFIRMED THAT RECORDS THAT ACCUSED ME OF HARASSMENT WERE BEING HELD UNDER ‘CATEGORY 2 VIOLENT AND SEXUAL OFFENCES’ FOR AT LEAST 10 YEARS AND QUITE POSSIBLY UNTIL I AM DEAD BECAUSE I AM SOMEHOW DEEMED TO BE A RISK TO THE PUBLIC. WHY? BECAUSE I SENT SALLY MUNGALL A LETTER AND A COMPLAINT DOCUMENT, IN GOOD FAITH. I EVEN GAVE HER EXPICIT INSTRUCTIONS TO INFORM ME IF SHE DIDN’T WANT ME TO CONTACT HER. SHE NEVER INFORMED ME. I NEVER SUSPECTED MY CONTACT WITH HER WOULD LATER BE DEEMED AS ‘UNWANTED’. SO NOW HAMPSHIRE CONSTABULARY HOLD RECORDS ON THEIR DATABASE UNDER NAME, ACCUSING ME OF VIOLENT AND SEXUAL OFFENCES BECAUSE I SENT SOMBODY A POLITE AND RESONABLE LETTER THAT HAS BEEN DEEMED ‘UNWANTED’.
SALLY MUNGALL WAS ACTUALLY OBLIGED, UNDER THE ETHICS AND STANDARDS OF HER OWN REGULATORY BODY, THE HEALTH AND CARE PROFESSIONS COUNCIL, TO RESPOND TO MY CONCERNS. SALLY MUNGALL, INSTEAD, SOUGHT TO USE THE STATE POWER OF THE POLICE TO ABSOLVE HERSELF OF HER PROFESSIONAL OBLIGATION.
HAMPSHIRE CONSTABULARY ACTUALLY HAVE THREE ‘OCCURRENCES’ RECORDED AGAINST ME. TWO OF THOSE ‘OCCURRANCES’ WERE, IN HAMPSHIRE CONSTABULARY’S OWN WRITTEN WORDS, ‘DUPLICATED’ AND SO RECORDED IN ERROR BUT AS FAR AS I AM AWARE, THOSE DUPLICATES, EMBELLISHED WITHOUT FURTHER EVIDENCE, REMAIN ON THEIR DATABASE.
THIS IS AN OUTRAGEOUS INJUSTICE.
MY PROTEST AGAINST THIS INJUSTICE IS RIGHTEOUS.
THE WORDS THAT YOU USED ‘INCIDENT NAMED BY POLICE AS STALKING FOR A HEALTH WORKER’ ARE MUCH THE SAME WORDS BEING USED EVERY TIME THERE IS AN MDT MEETING AT CMHT THE BRIDGE CENTRE IN OPENING STATEMENTS WHENEVER I AM BEING DISCUSSED.
THIS HAS TO STOP...
…I WILL BE WRITING A SECOND LETTER TO ADDRESS THE IRREGULARITES IN MY MEDICAL RECORDS. I WANT CONTEXT APPLIED TO QUALIFY ALL FUTURE STATEMENTS THAT REFERENCE ME BEING ‘REPORTED BY THE POLICE FOR STALKING’. THAT QUALIFIER WILL STATE THAT THE ACCUSATION OF STALKING IS MADE TOTALLY WITHOUT EVIDENCE AND ROBUSTLY DISPUTED AND DENIED BY ME…
…WITHOUT CONTEXT THESE WORDS IMPLY THAT I AM SOMEHOW ACTUALLY GUILTY OF STALKING. WHEN USED IN THE CONTEXT OF THIS PREVENT REFERRAL IT IS ALMOST CERTAINLY UNDERSTOOD BY THE READER AS A STATEMENT OF GUILT AND INFLUENCES DECISIONS BEING MADE BY THOSE CONSIDERING THIS REFERRAL.’
I WANT ALL FUTURE STATEMENTS THAT REFER TO ACCUSATIONS OF ‘STALKING A HEALTH PROFESSIONAL’ THAT ARE ENTERED INTO MY MEDICAL RECORDS TO BE QUALIFIED BY MY OWN STATEMENT OF DECLARED INNOCENCE.
My medical records are peppered with such irregularities, falsehoods and grievous misrepresentation. The authors of such misrepresentations have often made untruthful statements knowingly and cynically. On other occasions such untruthful statements have likely been made as a result of carelessness. Whatever the cause or intent, the impact upon me has been truly devastating.
